Over time, this powerful combination makes for great results. Functional medicine and neurology is a process of changing underlying conditions in the body and it takes time. In today’s world of immediate gratification, this can be difficult for many. Medication can sometimes offer immediate symptom relief, but it usually does nothing to fix the cause of the problem. That being said, patience and persistence are key on both sides of this relationship.
Phase One: Complete Case Evaluation
The Case Evaluation is the quickest and most efficient way to determine the underlying causes of your health concerns and to design a complete care plan to move toward resolution. We recognize this visit typically involves a greater time and energy investment than a typical visit with your medical physician. It is our experience that the case evaluation will yield a significant return on your investment of time and money when looking to your current quality of life and future. We ask that you complete the forms on the right side of this page prior to your visit. (Adults fill out 3 of them, kids only the intake) Make sure to bring these forms with you to your appointment along with all recent lab and genetic results and any relevant diagnostic imaging (X-rays, MRIs, etc.) and electrodiagnostic studies.
Your Case Evaluation Includes
A complete health history
Evaluation of previous diagnostic and blood tests.
Careful functional examination
Recommendations, requisitions and instructions for additional tests.
Phase Two: Report of our Findings & Program Design
This is the beginning of your education process.
We discuss how the information obtained from your case evaluation (phase 1) explain what we see on your test results. Tests results are discussed in detail and are made understandable and we will outline the path forward. You will understand the phases of care, how many phases we recommend for you, how long it can take and what type of diet and lifestyle adjustments you will have to make.
Phase Three: Ongoing Care
Choosing to move forward with our recommendations means you will utilize our services to guide you through the recommendations. This will include supplying you with the proper nutraceuticals needed, providing specific guidance and support (Including but not limited to, follow up appointments with doctor and staff, emails, written materials, video coaching support etc.)
Retesting in order to track changes to blood and body chemistry and to guide ongoing recommendations and adjustments to your process and to outline long term recommendations.
Outside of the NY Tri-state Area? No Problem
In addition to seeing patients in his NY office, Dr. Mike works with people from all over North America via Skype. His Skype address is Futuredoc.1. Call our staff to set up the appointment being mindful of time zone changes (we are EST). Fax any recent labs to 631-673-7055 or email them directly to Dr. Mike at firstname.lastname@example.org. We do two initial Skype appointments, the first is to go over the history and any recent testing and treatment you have done. At that point, Dr. Mike will order lab tests to fill in the gaps. Once the testing is complete, you will receive a call to set up the follow up consultation to go over the results and the strategy he creates for you. Then you meet for 30 minute follow ups either weekly, every other week or monthly depending on the case.
